Webbased form for membership site/user information by longsack

My site is using CMS Pro and Mach Forms Once inside the members area the site has forms that the members will fill in information and also upload some files. Currently the form sends emails to me and will not save the information for each user... (Budget: $30-$250 USD, Jobs: HTML, MySQL, PHP)
