Wordpress setup by caratee

Need someone could help me to do few plug-in. 1. need to fill in submission form before downloading trial service, able to add/delete/update. 2. once form is submitted, will automatically divert to a specific link, link will never be published... (Budget: $30-$250 USD, Jobs: HTML, MySQL, PHP, Wordpress)
